3. Photocopy of certificate of eligibility/rating/license; and
4. Photocopy of Transcript of Records.
5. Photocopy of Certificates from trainings/seminars attended; and
6. Proof of employment such as Service Record, relevant experience acquired through designation cover by a Special Order or Memorandum, Certificate of Employment, and/or Copy of Contract
